How to apply for a Hilldale home

We provide safe, suitable homes for vulnerable adults aged 18 and over. Our tenants have different care and support needs. This includes people with autism, people with learning disabilities and people with mental health needs. Our aim is to help each person live as independently as possible in a home that is right for them.

Could a Hilldale home be right for you?

You may be considered for a Hilldale home if all of these apply:

• You are aged 18 or over.
• You have care and support needs that have been assessed by Adult Social Care or health services.
• You have commissioned support hours in place. This means Adult Social Care or health services have agreed and arranged the support you need.
• You need a supported home to help you live safely and as independently as possible.
• A relevant professional from Adult Social Care or health services makes the referral for you.

You, your family or your support provider cannot apply directly to Hilldale. If you are interested in a Hilldale home, speak to your social worker, care manager or health professional. They can discuss your housing needs with you and make a referral if a Hilldale home may be suitable.

What happens next?

Speak to your social worker, care manager or health professional if you think a Hilldale home may be right for you. They will check that your care and support needs have been assessed, commissioned support hours are in place and then make a referral for you.

We will review your needs, preferred area, accessibility and safety information to decide whether we have a suitable home. If we can offer you a home, we will explain the tenancy, rent and service charges and agree when you can move in.

If we cannot offer you a home, we will explain why and you can ask us to review the decision.
